Description:
Transportation:
Offering ground transportation services by volunteer drivers for cancer patients and/or family members. Will travel to Minneapolis (MN), Saint Paul (MN), La Crosse (WI), Rochester (MN) and Decorah (IA). Patients must be ambulatory. Require 48 hour advance notice. Fees are suggested donations for Seniors (people over 60). Local rides (8 miles or less) are $5.00 round trip. Rides over 8 miles are $1.02 per mile. Those on MA or enrolled in Blue Plus or UCare, the county is billed directly. Volunteer drivers driver their own cars. They can take people in wheelchairs if the person can transfer themself in & out of the wheelchair.

Utility Assitance:
Energy Assistance Program (EAP): provides financial assistance to renters or homeowners to pay heating bills. Services include: Provide direct payment to the energy supplier. Educate consumers to use home heating energy efficiently and safely. Advocate with energy suppliers and human service providers on behalf of consumers. Crisis help for utility disconnections or necessary fuel deliveries. Emergency heating system repair or replacement. Must be resident of Dodge County.
Weatherization Assistance Program (WAP):Reduces energy costs for households by making improvements to the home that will reduce energy bills. Priority is given to households with at least one elderly or disabled member and to customers with the highest heating costs. Services available: Participant education. Energy audits to evaluate home's energy usage. Exterior wall and attic insulation. Air infiltration and bypass sealing. Test, repair, and/or replace home mechanical systems to ensure efficiency and safety. Must be resident of Dodge County.
Reach Out For Warmth Program: Assists households who do not qualify for either program and meet a higher set of income guidelines.

Patient Advocacy:
Provides personal support to individuals in need. Strives to support and empower family and informal caregivers by addressing their individual needs and preferences, reducing their burden and extending the time that care can be provided at home. The Advocate will provide information to assist with accessing health and community based services, help caregivers to identify and meet their needs; counsel caregivers and connect them with community resource; assist with completion of applications for assistance and/or services and provide ongoing contact for continuing support. They specialize in Medicare, Social Security, and Prescription Assitance programs. Provides patient advocacy for adults age 60 and over and people with disabilities in Fillmore & Houston Counties.
