Description:
Hospice of the Twin Cities offers comprehensive care focused on easing the physical, emotional and spiritual pain that often accompanies terminal illness. Offering outstanding care, the hospice team members are dedicated professionals who specialize in meeting the individualized needs of terminally ill patients and their families.

The Attending Physician remains the primary resource directing the care provided for the patient, while the Hospice Medical Director serves as a consultant to the Hospice Team.

Hospice Nurses provide individualized care focused on providing comfort, managing pain and symptoms, coordinating all hospice services and educating the patient and family to ensure the quality and continuity of care.

Social Workers provide psychosocial and emotional support for the patient and their family members and coordinate community services and resources.

Home Health Aides and Homemakers assist patients with personal care and light housekeeping.

Board-Certified Music Therapists address the physical, emotional and spiritual needs of patients and families through the utilization of music within the therapeutic relationship.

Pet Therapists provide a unique intervention increasing socialization and enhancing quality of life for patients and their families.

Trained Volunteers provide a variety of services focused on companionship and support for the patient and respite the caregiver.

Chaplains provide emotional and spiritual support to patients and families honoring the sacred dimension of end-of-life care, for each person's unique journey.

Bereavement Counselors assist the family with grief and loss issues for 13-18 months following the death of the patient.

The Hospice Team coordinates the provision of medical equipment, supplies and medications for pain relief and symptom management related to terminal diagnosis.

On-Call services are provided by the hospice nurses 24 hours a day, every day of the year.

Additional services including therapies, respite, inpatient and continuous care are coordinated by the hospice team, as needed.

Hospice of the Twin Cities offers care to residents of long-term facilities who are facing a terminal illness. In partnership with the long-term care facility staff, the hospice team provides state-of-the-art symptom management, as well as emotional and spiritual support for residents and their families.
