Courage Center’s Home and Community Rehabilitation works with many disciplines toward increasing a person’s functional independence at home, work, school and in the community.
We bring our rehabilitation team to you, in your home or in community settings to reintegrate what you learn in therapy to your "real life". Your therapy program is determined by your individual needs and potential. Each person is unique and goals are established with your input. Your team includes you, your family/concerned others, your physician, therapists, nurses, clinical coordinator and insurance case manager. Your team works together for a common goal: your increased independence.
Typical treatment settings are your real world including the places where you need to be able to function independently: the stores where you shop; the schools where you learn; the neighborhood where you live; the place where you work. These are your home and your community. This is where life happens.
