A health care directive—sometimes called an advance directive or living will—details your personal, emotional and spiritual needs regarding your medical care. It lets you name the person you want to make your health care decisions if you are not able to make them for yourself. Be sure to bring this very important document with you upon being admitted to Ridgeview.
The Five Wishes® advance directive lets you specify exactly what you want in an easy-to-use format. By completing this important document and bringing it with you when you are admitted, you control something very important: how you are treated if you get seriously ill.
