Description:
Offers transportation and case management for medical appointments, generally within an 80-mile radius of Mankato (this radius includes Rochester), but also to the Twin Cities from Mankato and the surrounding area. Use 5 company cars for all trips, but also accompany people who use an AMV van to medical appointments. Social workers or RNs help to manage the case, keeping records and talking with the doctors, etc. Charge an hourly rate and also mileage where transportation is provided. Assistance with emergency medical visits are available.

Additional services include: accompanying clients to appointments, assessments, availablity in emergencies, case management, coordinatoing appointments, crisis intervention, education, guardianships, health care advance directive information, identifying and coordinating area resources, Medicare and insurance assistance, respite, compansionship, errands, and home management tasks.
