Palliative and supportive services to dying persons and their families, provided by an interdisciplinary team addressing physical, social and spiritual care.

  • Medicare and Medicaid certified
  • Licensed in Minnesota and Wisconsin
  • Primary care physician involvement
  • Interdisciplinary team of RNs, LPNs, social workers, chaplains and clergy, home health aides, homemakers, pharmacists, nutritionists, volunteers, and the patient and family
  • Respite care
  • Homehealth aide/homemaking services
  • 24-hour on-call nursing support
  • Regularly scheduled home health aide visits
  • Medical equipment coordination
  • Pain management
  • Bereavement support
  • Patient and family education
  • The Gathering at Boutwells Landing specialized care suites
